Canberra Old Oak Surgery will be hosting a Health & Wellness Open Day for all members of the public on Thursday 17th November 2016 between 2.00pm and 4.00pm – all are welcome to attend. See poster below.
The event aims to raise awareness about and allow patients to experience a number of local health and social care service providers accessible through the surgery or by self-referral for our patients as well as other residents of Hammersmith & Fulham.
Visitors will also have the opportunity to meet PPG representatives and members of practice staff, particularly newer team members, and will be treated to FREE mini health checks, whilst younger visitors can enjoy FREE face painting.
The programme includes Healthwatch H&F who enable local voices to influence the delivery and design of local services.
Also due to attend:
Sobus provides support to local voluntary and community sector organisations, as well as direct support to communities in the borough, focusing on North End and Old Oak neighbourhoods. Also in attendance will be White City Enterprise who work to bring about improvements to the quality and efficiency of public services within the White City Opportunity Area, including through projects such The Parkview Community Champions Project, which uses a dynamic community engagement process to bring people together to build strong connections between communities and local services.
CLCH Family Planning and Sexual Health services provides a free and confidential range of services for women and men of all ages, and they will be present in addition to Nubian Life who deliver support for African Caribbean Elders, living with complex health conditions such as Dementia, Alzheimer’s disease, Diabetes, Sensory and Visionary Impairment and Physical Disabilities.
IAPT Back on Track is a free NHS evidence-based talking therapy service for people 16 years old and over, who are worried or have low mood and they will be present in addition to the Community Diabetes Team who provide support for people over the age of 18 living with type one and type two diabetes, in addition to coordinating specialist support services such as nutrition, foot care and signposts to retinal screening for house bound patients and patients with learning difficulties.
MEND, being short for ‘Mind, Exercise, Nutrition…Do it!’ runs a series of free specialist sessions to help families stay active and healthy in Hammersmith & Fulham, to promote healthy, active lifestyles and good nutrition in the borough. Connecting Care for Children (CC4C) is changing way the children’s care is commissioned, delivered and experienced by making the expertise of paediatricians in hospitals much more widely available.
Our local Old Oak Community & Children’s Centre brings together a range of services for children under five and their families such as family support, health and education and special educational needs.
The Carers Network aims to make a positive difference to the lives of unpaid carers in the diverse communities of Hammersmith & Fulham by providing them with a range of services, support and recognition, and Age UK Hammersmith & Fulham aim to maintain older peoples independence allowing them to stay safe and healthy.
Kick-it are the local stop smoking service who can provide you with six, free, weekly sessions of help and support to get you through the hardest part of quitting.
A number of other services will be present for the whole family to benefit from.
AT Medics practices schedule regular PPG meetings with SMS reminders, dedicated PPG notice boards and a translator if necessary, and are delighted to be hosting this event at Canberra Old Oak Surgery.
